- 2 Professional Cleanings, 2 Periodic Exams, 1 Emergency Exam, and all necessary X-rays.
- Savings Advantage: You receive 15% off all work.

The Perio-Maintenance Plan
Best for: Patients managing gum disease
Gum health is the foundation of your smile. If you have a history of periodontal disease, you require more frequent care. Most insurance only covers two cleanings; our plan covers four specialized maintenance visits.
Includes:
- 4 Periodontal Maintenance Cleanings, 2 Periodic Exams, 1 Emergency Exam, and all necessary X-rays.
- Savings Advantage: You receive 20% off Scaling & Root Planing (SRP) treatments and 15% all other work.
PROGRAM GUIDELINES
- No other discounts with this offer.
- Membership is not for re-sale.
- Membership valid for one year after redemption.
- Patient's portion of bill due at time of service.
Program Exclusions & Limitations
This plan is only honored at Lakeview Dental and is NOT an insurance plan that can be used with any other dental office. It cannot be used:
- In conjunction with another dental plan
- For services for injuries covered under workman's compensation
- For referrals to specialists
- For hospitalizations or hospital charges of any kind
- For costs of dental care which is covered under automobile medical
- In conjunction with coupons or cash discounts

To understand the value, let’s look at a common procedure: a Zirconia Crown. In the Orlando area, a high-quality crown typically averages around $1,400.
Why the Membership wins:
While the PPO total looks lower, that insurance company now has a $0 balance left for you. If you have an emergency in October, you pay 100% out of pocket. With the Lakeview Plan, you still get 15% off every single visit for the rest of the year, with no limits.
